1 The digital sensor was positioned backwards in the mouth and the incorrect side of the sensor was exposed to the incoming X-ray beam. The exposure surface of rigid charge-coupled device (CCD) digital sensors is flat, whereas the opposite surface of the sensor is not flat, as it includes the wire insertion point into the sensor, making the occurrence of this artifact infrequent.

2 Although rigid CCD digital sensors cannot be bent, phosphor plates can, and similar to traditional dental film, where the emulsion of the film can be damaged by rough handling – bent, creased with a sharp fingernail when removing the packet from the film, scratched with an X-ray clip, or damaged from the cusp of a tooth – phosphor plates can also be scratched, creased, bent or folded, and these artifacts will reveal themselves when the plates are scanned, compromising the image quality
