Is culture and AB always recommended?
Nsais?
Topical treatment?
You can rub in elastopad from Baddaky once daily to soften feet and protect. Gently cut away the hyperkeratotic outgrowths. Looks like he’s nibbling on his skin so I would assume he’s got some discomfort. Do dental rads of the paws incl carpus/tarsus to check for arthritis or bony changes. Weight loss and metacam 0,6 mg/kg. If prolonged metacam use is indicated I would also give famotidine 0,5 mg/kg sid.
